Over the past 10 years, the government has been required to support 20% of the estimated health insurance premium revenue, which is the legal standard for national health insurance. However, it has only supported an average of 14% per year. As a result, a total of 19.4 trillion won was insufficiently supported, and a shortage of more than 1 to 2 trillion won has been continuing every year. The government has been inadequately supporting national treasury support by calculating the estimated health insurance premium revenue lower than the actual amount, and the proportion of support from the Health Promotion Fund formed by tobacco surcharges is also decreasing due to legal restrictions. The problem of insufficient health insurance finances is becoming more serious due to the increase in the scale of national projects such as the conversion of upper-level general hospitals, the operation of emergency medical systems, and the support for essential medical care. Experts argue that the legal standards should be adhered to and national treasury support should be expanded, and they have proposed measures such as using the confirmed budget figures instead of the estimated revenue or abolishing the sunset clause regulations.
